Application Process
We currently only accept hard copy applications. You can download the general application package or rehabilitation package below and submit it to our office in person, fax, or by mail. Upon receiving your completed application, we will process it, which includes 3rd party verifications on all statements. Once your application is processed, we will notify you of your program eligibly status.
